[WSBAPT] Leaving real estate to unmarried partner in RLT

Client has three pieces of real property he wants to leave to his unmarried
partner through his revocable living trust. Should I list each piece as a
separate specific gift, and how much detail do I need to list (address, tax
ID, legal description)?
