[WSBAPT] Appointment of Notice Agent Under RCW 11.42.010 - TEDRA Agreement

Does anyone have (and is willing to share) a template for a TEDRA Agreement
appointing a nonprobate notice agent as contemplated by RCW 11.42.010(1), so
I don't have to reinvent the wheel? Please feel free to respond off list.
