[WSBAPT] King County Superior Court Will filing coversheet

I find myself in possession of a number of original Wills that I need to file in King County Superior Court as I can no longer locate the makers. Does anyone have an appropriate coversheet and/or other documents to get these filed or can otherwise provide guidance as to the process? I have never had to do this before.
