[WSBAPT] information on death certificates?

On the death certificate there is a section for "Informant" - that is who
provides the information.

Now if that person did not provide what is written, My guess is that there
is a template that has placeholder language. Example - in my documents for
the witness block I have Jessica and Roger Rabbitt, living in Toon
Town.....and then it is highlighted in YELLOW so that as I am preparing to
print or send, I will quickly catch that I have not input the witness
information. I might wonder if the funeral home or wherever they ordered it
has something similar that just got missed.

I have not encountered any errors as of yet so I cannot advise other than
to reach out to Vital Statistics.
