[WSBAPT] client can't sign Medical Directive

I have a client who can not physically sign a Medical Directive in front of two witnesses.
There is a special statute for Notaries authenticating documents when the signator asks someone else to sign for them RCW 64.08.100
But I am not aware of any way around this problem on a MD?
Any ideas?
