[WSBAPT] Lack of Probate Affidavit - Chase Approved?

If you mean a small estate affidavit, the bank sometimes provides its own form; and you must wait until 40 days after death.  I’m attaching one that I’ve used in the past, but it’s been a while, so check the statute for current requirements.

I have a client who lost her step-mom this past week.  She has just one account at Chase that has only $3,000. 

 

Does anyone have a Lack of Probate Affidavit that Chase has accepted that you would share?
