[WSBAPT] notice of life insurance lapse COBRA

PC asked about lapsed life insurance on her husband.
Husband and Wife move to Wa from Calif. Husband leaves his job which
included a life insurance benefit, for which he contributed premium payments
every month.
Wife says the COBRA notice did not include anything about continuing life
insurance.
Husband died recently; wife made claim and carrier says we sent notice with
the COBRA notice.
 
Out of my area.
Anyone interested? Would likely be a contingent fee. The benefit was
$750,000.
