[WSBAPT] Pleadings to establish UTMA

I have my first Order to establish an UTMA account.  The monies are from an estate to a minor and the court approved the UTMA and custodian today.
As administrator of the estate, I was ordered to open the UTMA blocked account in the name of the custodian (grandma of minor).
I've reviewed RCW 11.114 and it is clear as mud.

Does anyone have samples of pleadings to establish and file the UTMA documents or where I can obtain information to direct me on opening the case "4" at King County Superior Court for filing of receipt of funds into blocked account establishing the UTMA?  Thank you.
