[WSBAPT] Fidelity error in failing to add beneficiary to IRA account

I have a Personal Representative with a similar issue with Ameriprise,
resulting in loss to an estate of about $30K.  I would be interested in

A client had Fidelity fail to add a beneficiary designation despite the
request to do so on Fidelity paper work.  Loss is stretch out of ten years
or more on a $500,000 IRA.  I am wondering if any members have litigated
with Fidelity or other brokerage firms on a similar issue.  Allen
