[WSBAPT] Distribution of minor's share to blocked account

I represent the PR of an estate in which one of the beneficiaries is a
minor. The minor's share of the estate is relatively small (less than 15k).
There don't appear to be any trustworthy persons available to serve as a
custodian under a UTMA account and the PR doesn't want to serve as custodian
herself.   Fortunately, another option exists. RCW 11.76.095(1) provides
that a minor's share can be distributed to an account "for the benefit of
the minor subject to withdrawal only upon order of the court in the original
probate proceeding, or upon said minor's attaining the age of eighteen years
and furnishing proof thereof satisfactory to the depositary." Anyone know of
particular banks that will or will not agree to such conditions? Does anyone
have a form they are willing to share to have the bank sign?
