[WSBAPT] Order blocking financial accounts

I have never had the (mis?)fortune to need to block an existing account,
but to create a blocked account in the first place, I recently just used an
order that had nothing other than the requirement from the Court that an
account be a blocked account. I would imagine that all the bank would
require is the identity of the bank, address of the branch, name on the
account, and account number, plus the requirement that the bank block the
account, but perhaps I'm missing something that differentiates a
modification to an existing account from the creation of a new blocked
account.
