The insurance company just confirmed that they changed the beneficiaries!

Hi, all,

 

In 2011, an annuitant sent a change of beneficiary form to his insurance
company for his annuity.  His signature was witnessed by a bank officer who
knew him who also faxed it for him.  Recently the attorney-in-fact for the
annuitant was reviewing his assets and saw that the beneficiary had not been
changed.  An insurance company rep initially told the AIF that the change
had been received but that now it was too old to process (really).  Now they
say the COB form was never received.  

 

The annuitant is very ill and not mentally able to sign documents.  Does
anyone have any suggestions on dealing with the insurance company?
