[WSBAPT] need example/template - petition for custodianship

Hello,

 

Does anyone have an example of a petition for custodianship that they are
willing to share? In this case, I am petitioning so that a parent can
become the successor custodian for some funds in an investment account
that were left to a minor by a grandparent.
