[WSBAPT] Trust Beneficiary Termination form

I was wondering if anyone would share a  form for a beneficiary to sign
waiving claims against a trustee and acknowledging receipt of funds and
termination of a trust.  Client has been trustee of an intervivos trust
set up by beneficiary’s grandmother.  Client/trustee is beneficiaries’
mom.  Beneficiary is now 40 which was the age the grandmother wanted him
to receive the remaining corpus of the trust.  This started before the
trust statute changed which included the notice to beneficiary’s of the 3
year period to make claims.
 
I could just re-work an estate beneficiary’s receipt and waiver form, but
I thought I would see if a better form existed.
