[WSBAPT] Termination of trust & holdharmless form

I have a client who has been a trustee for her son which was set up in
grandmother’s trust.  He has now turned 40 and the trustee needs to
distribute remaining funds to him.  Trust has been going on for several
decades.
 
Could someone share with me a form they use for a beneficiary  to sign,
acknowledging receipt of funds and termination of trust, waiving notices,
and holding trustee harmless.
This is a situation where everyone is getting along fine, I just want the
trustee to close the trust down properly.
