for granting financial support to organizations providing community services on

complete the required enclosed application form and return all materials to
me. This form is designed to provide only summary information to
facilitate a rapid overview by the City Council of each request; however, you
should provide the supporting documents identified on the application form as
attachments. In addition, any brochures and other information you
feel would be beneficial to the Council in their evaluation process should be

is the <b><span style='font-weight:bold'>absolute deadline</span></b> for
filing this application to receive consideration for funding during the
City’s fiscal 2006-2007 budget. The City Council will meet in a
workshop scheduled for <st1:time Minute="0" Hour="17" w:st="on">5:00 p.m.</st1:time>,
October 23, 2006, to review the applications and to listen to organization
presentations. Following these presentations, the Council will allocate
the available funds ($19,000.00 total) to successful applicants. <b><u><span
style='font-weight:bold'>At the October 23<sup>rd</sup> meeting, each applicant
will have approximately five minutes to present their application. Your
attendance at that meeting is strongly encouraged.</span></u></b><o:p></o:p></p>

certain services can be more effectively provided by contracting with outside

requesting funding consideration must meet at least one of the following three

activity which requires seed money for a single event and occurs within
one budget year;<o:p></o:p></span></font></li>
<li class=MsoNormal style='text-align:justify;mso-list:l0 level1 lfo2'><font
size=2 face="Book Antiqua"><span style='font-size:11.0pt;font-family:"Book Antiqua"'>An
activity which has the potential of becoming self-sufficient. This
type of activity could be one just starting which has an obvious funding
need and which has a constituency that appears to have the potential of
financially supporting the activity within a short period of time.
An example is a three (3) year grant which declines annually such that in
the fourth year no further funding would be provided;<o:p></o:p></span></font></li>
<li class=MsoNormal style='text-align:justify;mso-list:l0 level1 lfo2'><font
size=2 face="Book Antiqua"><span style='font-size:11.0pt;font-family:"Book Antiqua"'>An
activity which provides a specific, on-going service to the City that
would be granted and contracted with the idea that this contract would
continue for an extended period of time. <o:p></o:p></span></font></li>
</ol>

outside organization or agency shall submit their request for funding
consideration on a form prepared by the City. Such request must be
submitted to the City Clerk prior to October 1<sup>st</sup> of each year.
Council will hear individual presentations in order to evaluate each
proposal. Council will make final funding decisions by November 30<sup>th</sup>
of each year. <o:p></o:p></span></font></p>
<p class=MsoNormal style='text-align:justify'><font size=2 face="Book Antiqua"><span
style='font-size:11.0pt;font-family:"Book Antiqua"'><o:p> </o:p></span></font></p>

entities which have secured a grant through this process will complete and sign
a statement indicating that, as a condition of accepting these funds, they will
comply with all City policies regarding nondiscrimination and in addition, will
submit an accounting of how funds were distributed. This accounting will
be submitted to the City Clerk prior to May 1<sup>st</sup> of each calendar
year. Failure to do so may eliminate the entity from future grant
consideration. <o:p></o:p></span></font></p>
