I am writing to you to express my opposition to proposed cuts to the Mental Health programs in Idaho, specifically the PSR Program.
 
I have been a licensed social worker for over 40 years and am now retired.  I worked for the State of Idaho as a CPS Social Worker and Supervisor and owned my own PSR Case Management agency for eight years before my retirement four years ago.
 
Here’s a brief history of mental illness treatment.  Before the 1970’s mentally ill persons were largely confined to large, inefficient, expensive institutions such as the Mental Hospital in Orofino.  When miraculous advances in drug therapy came along in the 1970’s the mental health emphasis switched to community based treatment at mental health centers with patients receiving drugs for treatment of symptoms.  Then, in the 1990’s private home based agencies sprang up to provide patients with supervision so they could live in their own homes without hospitalization mostly through Case Management services (CM).  Then, in the 2000’s the state of Idaho minimized CM services and switched to more expensive time consuming treatment called PSR.
 
I liked the Case Management program.  It worked well.  I was able to provide care for mentally ill patients.    I was able to visit clients from the extremes of once a month up to four hours a day.  I helped them with landlords, courts, assured they took their medication, & kept them out of jails and hospitals.  Using mostly bachelor’s level social workers it cost the state an average of $500 a month for my clients
 
Specifically, one time I was with a client in a doctor’s waiting room and the radio was broadcasting a description of a rapist.  I asked my client if he knew anyone that fit that description and he gave me a name.  I handed him my cell phone and he called the police and that rapist is now serving 25 years in jail.  Another time, a client told me some bad people had taken over his apartment.  We contacted the Kootenai County Drug Task Force who raided the place and found a meth lab; later they arrested the men who had tried to manipulate my client.
 
When the state of Idaho cut back CM services drastically in about 2000 the client I mentioned above was dead within two months because I was unable to visit him daily.  In fact, I heard from a colleague that the suicide, hospitalization and incarceration rates went up significantly when the CM program was cut.
 
I’ve heard it mentioned that volunteers could help the mentally ill and make up for a cut in services.  Trust me, the little old lady or man from the local church is not equipped to deal with the time and energy and knowledge it takes to help the mentally ill.  I had one client who was mentally ill, autistic and retarded who called me at least 20 times a day for two years, usually repeating the same inane questions.  Most people would find this intolerable.  In fact, I had a registered Nurse employee who quit because the mortality rate of clients following the cutback in CM services was depressing to her.
 
Also, keep in mind that there is a federal match for CM and PSR services under Medicaid, so I think Idaho loses $2 for every $1 they save in Medicaid cuts.
 
Have you noticed that in North Idaho and Spokane there have been at least eight police related shootings of people in the community?  I believe this can be directly related in cuts or lack of services for mentally ill patients.  Do you recall the shootings recently in Tucson where Congresswoman Giffords and about 20 other people were shot because an obviously deranged person and six people were killed?  I believe you’re going to see many more community problems like this if these Medicaid cuts in PSR go through.
 
In summary, paying $500 a month per client average for CM services a month is a lot cheaper than the state cutting PSR services and then ending up paying more in suicides, homicides, hospitalization and incarceration costs.  I recommend that you de-emphasize PSR and restore the 20 hours per week authorization for CM services.
