No. This is going to be in regulations -- it's not in the bill itself -- but
I suspect you'll receive something like a W2 from your insurer, which you'll
submit along with your  taxes or suffer a $695 / or 2.5% of income tax
 penalty

Yeah. There's no restrictions on doctors in the entire mandate section.
